The Oxford Lab Products Decontamination Form is used to protect the health and safety of Oxford Laboratory Products
Employees. We require that all products be fully decontaminated prior to return for warranty or repair. Please fill out this form
and submit it to firstname.lastname@example.org. The decontamination form must be submitted for review and authorization and reviewed
by the Oxford Product Manager. Once approved, the Product Manager will provide a signed and authorized form to accompany
the return. Return/warranties will not be accepted or fulfilled unless the decontamination form has been signed and authorized.
This form must be included in the return/warranty shipment with an Oxford Product Manager authorization.
